In order to initiate payment of the benefit, peculiar ZUS should be paid ERRD form – "Application for an orphan's full allowance".

The application shall be accompanied by appropriate papers to confirm the position of complete orphan. These are primarily:

  • Both parents' death certificates - in a standard situation.
  • Mother's death certificate and baby's birth certificate – where the father is unknown. The birth certificate must show a deficiency of father data.

The application together with the annexes can be made in respective ways: personally in any ZUS facility, via post, through a proxy or even from abroad through the Polish Consular Office. It's a large aid for people who, for various reasons, can't handle things personally in the country.
